This individual will be responsible to perform duties related to as the Intake as well as perform
general case management for halfway house level of care which includes; coordinating welfare contacts, scheduling medical appointments, determining client needs and setting up services such as employment, housing, and volunteering. Case Manager is also responsible for follow-ups, coordinating completion activities, coordinating graduation and presenting seminars on a variety of topics relevant to reconnecting to the community. Also interacts with funding sources, must be familiar with the PCPC and Utilization Review.
